If you already have a promotion code, and you are placing your order online or by phone, simply provide the code during the check out process. Or, if you are ordering by mail or fax, you can specify the code and discount amount using this Order Form. 

Shipping options

Standard Delivery
Price includes UPS Ground shipping. Unless an expedited service is requested, your order should arrive in approximately 5-7 business days after your order confirmation is received. Orders received during normal business hours are processed within 24 hours of receipt. Order placed after 11:00 A.M. (EST) on Friday, or during the weekends, will be processed Monday.

Expedited Shipping
Expedited UPS shipping is available for an additional charge of $25.00. This upgrade option is available for internet and phone orders only. Your order should arrive the next business day if placed before 11:00 A.M. (EST), Monday – Friday. Orders placed after 11:00 A.M. (EST) on Friday, or during the weekend, will be processed Monday, and should arrive the next business day.

Shipping to Multiple Locations
